It could be several things. The photo is hard to tell, it looks like some type of yellowish discharge is going on too. Ear hematomas are fluid filled and can be hard and hot or squishy. They need to be seen by a vet and lanced/drained in order to heal. With your pup being so young, really a vet is a good idea to get checked out.

Good evening, My name is John.My lab has an Aural Hematoma. The Vet prescribed my Pebbles with an antibiotic and steroids. At first the it was small at the bottom of her ear. Now it’s her whole ear.
Hello Caroline….you need to take your dog to the Vet. It sounds as if he has a very painful ear which needs the correct treatment. This really isn’t something you can treat yourself.
Please take your dog to the Vet with some urgency hes obviously suffering.
as we’ve said each time you’ve asked this question, this is something you can’t fix with oral meds. get to a vet.
There is no oral medication that can be given for a aural hematoma. Your dog needs to go to the vet.
